Observed two coolers above 41 degrees. Cold food items must be held at or below 41 degrees to prevent bacteria growth. 3- Cold held and bulk items not properly labeled. Observed labels and dates needed on cold food items being stored in coolers. Observed labels needed for bulk item containers. Food must be labeled and dated for easy identification and to prevent contamination. 4- No hot water at prep area handsink. Observed vent hood and oven in need of cleaning. 3- Observed door seal on walk-in cooler in need or repair or replacement. Door seal is falling apart from the middle of the door to the bottom. Re-inspection date: April 8.

Work order placed. 2- Observed back door left/propped open. All doors/windows must remain closed except when in immediate use. Corrected on site.

Observed ice buildup on piping of walk-in freezer. 2- Observed floor tiles missing in back hallway near mop sink, water puddled due to missing tiles. Also observed walk-in cooler door not properly closing. Will make corrections.

Holiday Inn Express and Suites, West Plains, violations: 1- Electrical wiring or wiring components do not meet minimum requirements as evidenced by GFCI not observed in room 201, 301, 401. Will install GFCI outlets in each room. 2- Observed emergency lighting on 3rd floor near ice machine and on 4th floor near room 421 not working. 3- Requirements for relief valve discharge pipes not met as evidenced by proper discharge pipe needed. Piping cannot be PVC pipe and needs to have 3 inches of clearance from ground. Follow up date: April 20.

Also observed cleaning needed at A/C vents/filters on the North & South side 2nd floor hallway. Observed wall repair needed in rooms 122 and 226, as well as in the storage room on the North side of the 1st floor. 2- Pests not appropriately controlled or minimized as evidenced by observing gap at the back door of the laundry room. 3- Electrical wiring or wiring components do not meet minimum requirements as evidenced by no power to the GFCI outlet in Room 134. Observed covers needed in the pool room for electrical outlets. 4- Required exit signs not maintained as evidenced by EXIT lights in the pool hallway and the North side of the 1st floor not in proper working order. 5- Observed Emergency lights not in proper working order between rooms 131 & 129, the North side stairwell, and between rooms 218 & 216. 6- Facility's smoke detectors and heat sensing devices not hardwired with battery backup in 2nd floor storage room. 6- Rooms containing gas water heater, dryers, chemicals do not have hardwired CO2 and smoke detectors.
