Ozark Pizza Company, West Plains, violations: 1- Observed ingredients & sauces not properly labeled. Items stored in prep coolers had no label or date of preparation. 2- Establishment’s plumbing in disrepair. Observed leak at prep handsink. Also, no paper towels available at prep handsink. Reinspection date November 30.
November 4
McDonalds, Mountain View, complaint response, violations: 1- Equipment not maintained in a state of repair or proper adjustment. Observed ice buildup in walk-freezer around the secondary door. 2- Observed cleaning needed throughout, specifically at drink station in drive thru, and the slush machine. Reinspection date December 4.
Los Amigos Restaurant, Mountain View, violations: 1- Food not protected from contamination. Observed food being stored uncovered in prep cooler and freezer. Also observed food being stored in improper container. Food being stored must be covered and in proper containers. 2- Failure to properly date label all refrigerated, ready-to-eat potentially hazardous food. Observed prepped food in walkin cooler not properly labeled or dated. Prepped food must be labeled and dated to prevent contamination. 3- Bulk items not properly labeled. Observed bulk items in food safe containers not properly labeled and scoops not properly stored. All food items must be labeled and scoops must be stored to be grabbed by handle without touching the food item. Reinspection date December 4.

Corrected onsite. 2- Bait stations not covered or tamper resistant. Observed uncovered traps. Will use concealed traps. Corrected onsite.

Subway #2, West Plains, violations: 1- No hot water at front prep handsink. The water temperature did not reach 100 degrees within a minute. Reinspection Date December 10.

McDonald's #2, West Plains, violations: 1- Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees. Observed soft serve and a cooler above 41 degrees. Temperatures must be maintained below 41 to prevent contamination. 2- Equipment not in use stored outside facility. Observed multiple carts with various items in hallway and to the back door. Reinspection date December 11.

Sip & Bite, Mountain View, violations: 1- Food not protected from contamination. Observed a bag of uncut onions being stored on the floor. Corrected onsite.

The Pasta Place, Willow Springs, violations: 1- Observed two bulk item containers not properly labeled. Corrected onsite. 2- Evidence of a mouse observed. Will clean and have pest control come in. Reinspection date December 13.
Jason's Mexican Restaurant, Willow Springs, violations: 1- Food not protected from contamination. Observed food stored in cooler and freezer uncovered. Food being stored must be covered to prevent contamination. Observed food stored without proper labels identifying the food and the date of preparation. Food must be properly labeled and dated to prevent contamination and bacteria growth. Reinspection date December 13.

Primo's Grill, West Plains, violations: 1- Hair restraints not being worn by all food preparation staff. Observed no hair restraint on cook. 2- Failure to maintain 41 potentially hazardous foods at a safe cold holding temperature of 41 degrees. Observed prep table with a holding temp of 65 degrees. Cold held items must be held below 41 degrees. Food moved to different prep hold. 3- No hot water at prep area handsink. Observed hot water turned off due to leak at faucet at the prep handsink. Reinspection date December 17.

Barkers Hilltop Package, West Plains, violations: 1- Unsafe food not discarded or reconditioned according to an approved procedure. Observed food in walk-in cooler and crackers on the shelf out of date. 2- Equipment not maintained in a state of repair or proper adjustment. Observed cleaning needed on the cooling system of walk-in cooler. 3- Floor to wall junctions must be coved and impenetrable, floor drains shall be in place and draining appropriately. Observed gap at back door. Door needs sealed to prevent pests from entering. Reinspection date December 21.

Taco Bell, Mountain View, violations: 1- Food not protected from contamination. Observed boxes stored on floor in walk-in cooler and freezer. Corrected onsite. 2- No hot water at front area handsink. Water at prep area handsink must reach 100 degrees within a minute. The front prep sink failed to do so. Corrected onsite.
