Ruby Tuesday, West Plains, follow up violations: 1- Food not protected from contamination. Observed foods in prep area freezers stored uncovered. Keep lids on product to prevent possible contamination. 2- Observed floor tiles in need of repairing. Work order in place and will be corrected prior to next routine inspection. 3- Observed oven in need of cleaning. Reinspection date September 28.
West Plains Motel, West Plains, not approved, reinspection date September 16, violations: 1- Baseboards in several rooms in need of repair. Room 2 baseboards, Room 17 and 45 carpet, several restroom floor tile broken or damaged. 2- Most rooms not properly cleaned between use. Shoes under bed in room 46, trash behind dresser in room 17. All rooms need deep cleaning. 3- Linens unclean as evidenced by bedding in need of replacement in room 33. 4- Observed mattresses stored outside. Must keep in clean, dry location. 5- Pests not appropriately controlled or minimized as evidenced by roaches and bugs located in rooms 17 and 33. 6- Garbage and refuse not properly maintained as evidenced by trash bags, empty boxes and bottles on the sidewalk, along with recliners and a/c units stored on sidewalks. 7- No working smoke detector located in rooms 16, 25, 33, and 44. 8- Facility's smoke detectors and heat sensing devices not hardwired with battery backup. No smoke detectors observed hardwired. 9- Equipment not maintained as evidenced by leaking plumbing in several bathrooms and mechanical room with leaves and debris in floor.

Primo's Grill, West Plains, Pre-Opening inspection due to change in ownership. Approved to apply for liquor license. Will Re-inspect in 3 weeks. Violations: 1- Observed food in hot holding below 135 degrees and a cooler above 41 degrees. Hot hold items must be held above. 2- Cold hold items must be held below 41 degrees. 3- The employee handsink is inaccessible. Various items were being stored in the sink, preventing employees from washing hands in the prep area. Also, one handsink did not meet hot water temperature of 100 degrees. 4- Observed damaged floor tiles in in kitchen prep area. Replace/repair as time allows.

Observed prep tables above 41 degrees. to prevent contamination, all cooling devices must maintain a temperature below 41. 2- Observed bulk items not properly labeled. Multiple bulk item containers did not have labels to properly identify the food item. The containers also did not have proper scoops for the food item when needed. Prepared items in cooler did not have prepared dates. Prepared food in coolers must be covered when not being readily used. 3- The employee handsink is does not provide adequate temperature for washing hands. The temperature after a minute did not reach 100. The faucet is also not firm on the sink. Reinspection Date November 6.

All food must be labeled and dated for proper identification and to be used in the appropriate time frame. Reinspection date November 20.

Observed no air gap on the 3 vat sink. Must backflow prevention. 2- Material used for storage doesn't meet guidelines. Shelving is not smooth and easily cleanable. Must paint or stain for a easily cleanable surface. 3- Observed holes in wall, gap at the back door, hole for air conditioner. All need to be repaired to prevent contamination and pests. Air vent needed in restroom.
