Willow Springs Elementary, Willow Springs, violations: 1- The vents not smooth and easily cleanable. Observed vents with rust, in need of painting to be smooth and easily cleanable.

Dennis' Specialty Cuts, West Plains, violations: 1- Improper thawing of Potentially Hazardous Foods. Observed meat being thawed by sitting in tub of water. Proper thawing methods discussed. 2- Observed ceiling tile missing in bathroom. Observed flooring in need repair. Reinspection date June 4.

West Plains High School Cafeteria, violations: 1- Excessive ice buildup on fan and piping of walk-in freezer. Reinspection date June 5.
West Plains Elementary Cafeteria, violations: 1- Hair restraints not being worn by all food preparation staff. Observed some staff not wearing hair restraint while preparing food. Reinspection date June 5.

Food must be stored in food safe containers. 2- Observed no paper towels available at prep area handsink. 3- Improper storage of toxics. Observed cleaning supplies stored above food items. Reinspection June 11.

Mtn. View Elementary, violations: 1- Observed tiles missing or broken in the dish wash area and near the ice machine. Reinspection June 11.

Taste of India, Mountain View, violations: 1- Improper use of single-service gloves. Observed gloves needing to be worn by food preparation staff. On-site for follow-up to priority. Will follow up at next routine. Will also check on core violations from previous inspection in March.

Pizza Inn, West Plains, on-site due to complaint. Violations: 1- Cold held food items not properly labeled. Observed cold held food items in prep table not properly labeled or dated.

Honeysuckle Inn, Mountain View, violations: 1- Observed vents in A/C units throughout needing cleaned with debris buildup on the filters. 2- Observed railing on the exterior hallway, 2nd floor, westside. One piece of railing not properly attached. 3- Observed EXIT lights not in proper working order throughout facility. 4- Observed emergency lights throughout needing replaced/repaired to be in proper working order. 5- Observed outlet cover needed in the electric room for exposed wires. Follow up date June 12.
Malone's Motel, Mountain View, violations: 1- Observed carbon monoxide detectors needed in the laundry room and near the water heater. 2- Requirements for temperature and pressure relief valves not met as evidenced by the need for copper galvanized pipe needed for the relief valve. Follow up June 12.

Observed hair restraint needed to be worn by some food prep staff. 2- Food not protected from contamination. Observed food being stored uncovered and in original metal containers. Food being stored must be covered to prevent contamination. Once a metal container is open food must be stored, covered in a food safe container. 3- Observed cold held food items not properly labeled. Cold held food items not properly labeled or dated. Food being stored must be labeled and dated for easy identification for discarding and to prevent contamination. 4- Excessive air gap. Observed gap at back door. Exterior doors must be sealed properly to prevent pest entry. Reinspection June 17.
