Mason's Midwest Meats, West Plains, violations: 1- Excessive debris buildup on cooler tracks and inside of cooler. Must keep cooler tracks and inside of coolers clean for food storage. Re-inspection date: 01/04/2026

Dairy Queen, West Plains, violations: 1- Unsafe food not discarded or reconditioned according to an approved procedure. Observed cut tomatoes and onions in walk-in cooler past the discard date. 2- No hot water at front prep area handsink. The front handsink did not reach 100 degrees within a minute. Re-inspection date: 01/07/2026

Willow Springs Laundry Snack Window, Willow Springs, violations: 1- Employee hand sink not located in the food preparation area nor is it convenient for employees. Observed no designated hand sink available. No paper towels were available either. Deadline for correction: 01/08/2026

Primo’s Grill, West Plains, follow-up, violations: 1- Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees. Observed prep cooler at 48 degrees. Items in cold hold must be held below 41 degrees to prevent bacteria growth. Re-inspection date: 01/18/2026.
